For RAND, this excludes "horizontally integrated" health systems, such as systems consisting only of hospitals. For its work, a health system must have at least one acute care hospital and at least one physician organization. The RAND Center of Excellence defines a health system as two or more health care organizations affiliated with each other through shared ownership or a contracting relationship for payment and service delivery. RAND Center of Excellence’s Definition of a Health System In addition, health systems are required to include at least 1 general acute care hospital, 10 primary care physicians, and a total of 50 physicians all located within a single hospital referral region.

The National Bureau of Economic Research (NBER) Center of Excellence defines a health system as a group of healthcare organizations (e.g., physician practices, hospitals, skilled nursing facilities) that are jointly owned or managed (foundation models are considered a form of joint management).
